What is Provenance and source history?
Provenance describes the origin of content and the processing steps leading to its current state. It can include source, version, transformation, responsible party and time. A citation is a starting point; alone it proves neither authenticity nor correctness.
For RAG, keep the answer traceable to the document passages used. For review evidence, preserve the relationship between the test, system version and result.
